Similar to the Appointment Verification form, this form is used to verify members’ health care appointments that happen at the same location on a consistent and reoccurring basis.

Grievance and Feedback Form

Online form to submit a grievance and/or other feedback about the Ride to Care program. Online submissions are monitored from 7 a.m. to 6 p.m., Monday through Friday. For urgent grievances please call Ride to Care directly at 503-416-3955 for immediate assistance.

Online form to cancel any trips that have already been scheduled. Please fill out form completely. Do NOT use this form if your trip is scheduled for today or tomorrow. Please call Ride to Care for same-day or next day cancellations at 503-416-3955.
